Step 1: Complete information about your location and the battery
Carefully complete information about your location and the battery.
Make sure contact details are correct as documents will be returned to the email address submitted.
Please Note: Once started you must finish with all module input. If you have to suspend input action you will need to repeat the evaluation in full again.
Answer the questions carefully – We strongly recommend this is completed offline first, and responses entered as below. A PDF of the evaluation form can be found in the documents tab.
Battery checks should be carried out by correctly trained individuals only. Please ensure the correct personal protective equipment is used.
If the battery is damaged, take pictures before completing the online evaluation form.
If the battery has DTC’s, record them before completing evaluation. Please see the list of DTC’s in the evaluation form in the documents tab.
After completing the evaluation there are 3 possible outcomes:
1. The battery is automatically evaluated as “GREEN”
2. The battery is automatically evaluated as “YELLOW”
This would result in all relevant documentation being automatically emailed back to you within 48 hours.
or
3. The Evaluation needs FURTHER REVIEW
If this occurs, responses will then be reviewed by our Ford Technical Experts.
This review should typically be completed within 5 working days and a response will be provided via email, including documents to support transportation.

If the evaluation relates to a Ford Field Service Action (FSA), you MUST have the Full VIN (17 characters) and FSA Number available before proceeding further. Both are mandatory inputs for the correct completion of the FSA evaluation.
